Signature Health to Open New Painesville Location for Behavioral Health and Medical Services

The new location at 54 S. State St. will open for patient care on Monday, May 9
April 20, 2022: Signature Health, a Federally Qualified Health Center (FQHC), will open a new, larger Painesville facility for outpatient care on Monday, May 9, 2022. Its current office at 462 Chardon St. in Painesville will close on Thursday, May 5, and staff will move to the fully renovated, new facility at 54 S. State St.
“Signature Health is proud to serve the Painesville community and increase access to high-quality, much-needed health care services,” said Jonathan Lee, CEO, Signature Health. “This beautiful new downtown location will make it easier for community members to address their behavioral health or medical concerns. We’re focused on removing barriers and continuing to expand our services to meet the evolving needs of this vibrant community.”
The three-story, 27,000-square-foot building will offer integrated care for patients of all ages, including mental health, addiction recovery, primary care, and infectious disease services. This includes:
- Individual, couples, family and group counseling
- Psychiatry services
- An Intensive Outpatient Program (IOP) for substance use disorder (SUD)
- Case management
- Primary care, for newborns through seniors
- Sexual and reproductive health services
- Prevention, free screening and treatment of infections including hepatitis C and HIV
- On-site lab services
- Walk-in assessments for new patients
- An on-site pharmacy that is expected to open in summer 2022

Signature Health receives funding and support from the Lake County Alcohol, Drug Addiction and Mental Health Services (ADAMHS) Board. ADAMHS Executive Director Kimberly Fraser said, “We’re very proud to have Signature Health as a part of our ADAMHS system. The agency enjoys a well-earned reputation for outstanding services and smart management. We’re confident this new and expanded presence in Painesville will be another important step for Signature Health, and significant benefit for our community.”
The new location offers ample free parking and easy access to Laketran bus routes 1, 4, 5 and 9. Signature Health primarily serves Medicaid, Medicare and uninsured patients, regardless of their ability to pay. A sliding fee scale program is available based on household income.
Signature Health also plans to launch dental services at the new Painesville facility in late 2022, thanks in part to a grant from the Delta Dental Foundation. “Oral health is an integral component of the whole-person care community health centers like Signature Health provide,” said Holli Seabury, EdD, executive director of the Delta Dental Foundation. “We’re proud to play a small part in creating a new dental home for the Painesville community.”
