Signature Health’s skilled and compassionate team in Ashtabula cares for both children and adults. We offer:
- Mental health care
- Addiction recovery programs
- Primary care for all ages
- Infectious disease prevention, screening and treatment

Appointments are available in-person at our office in the old Hotel Ashtabula (parking is free) or using video (telehealth).
To establish mental health or addiction recovery care, patients complete a 90-minute assessment to determine appropriate services. These appointments are typically scheduled in advance, or we offer some walk-in slots Monday through Friday. To schedule a new patient assessment, you can call the office, schedule online, or live chat a patient services team member during business hours.
Public Transportation
Ashtabula County Transportation System (ACTS) busses run from 8am to 4:30pm (Monday - Friday), and stop near Signature Health every hour, between 5-15 past the hour. Learn more about the routes and door-to-door service.
